Resolution:  

1. Navigate to https://www.office.com/
   
    
2. In the upper right corner of the screen click your profile picture
   
    
3. Click "View Account"
   
    
4. Look for the tile that says "Security Info" and click "UPDATE INFO"
    
5. Locate the correct device and click "Delete"
